This form is preprinted on both sides.
This form has a horizontal perforation and when the form is folded, written service notes and materials/products will transfer to the reverse side.
NO Location/Site/Method of treatment area on back side.
Please note that yellow and green are for indicating where the carbonless treatment is on the form only.
This form is blue and white.
